What Happens If You Miss a GLP-1 Dose? A Doctor's Guide to Semaglutide, Tirzepatide, and Restarting Treatment

You forgot your injection.

Maybe you were traveling. Maybe your schedule got away from you. Maybe you ran out of medication or simply couldn't make your usual injection day.

Now you're wondering:

Should I take it today? Wait until next week? Start over?

It's a question we hear from patients, and it's worth getting the right answer.

Missing one GLP-1 dose doesn't automatically mean you've ruined your progress or need to restart treatment. But what you should do next depends on which medication you're taking, how long it's been since your scheduled dose, and whether you've missed one injection or several.

Semaglutide and tirzepatide don't have identical missed-dose instructions. And taking medication again after several weeks off is a different situation from taking an injection one day late.

Here's what I want patients to understand before deciding what to do next.

What Should You Do If You Miss a GLP-1 Injection?

If you miss a GLP-1 injection, check the instructions for your specific medication before taking another dose. Some medications allow you to take a missed injection within a certain number of days. Others instruct you to skip it if you're too close to your next scheduled dose.

The first thing I want you to know is that you shouldn't try to make up for a missed injection by taking extra medication.

And don't assume that instructions for one GLP-1 apply to another.

For example, the missed-dose window for Ozempic is different from the window for Zepbound or Mounjaro.

The medication matters.

The timing matters.

And if you've been off treatment for an extended period, your previous dose matters too.

Missed GLP-1 Dose: Semaglutide vs. Tirzepatide

For FDA-approved, once-weekly injectable products, the prescribing instructions differ.

Medication What the prescribing information says
Ozempic (semaglutide) Take the missed injection as soon as possible within 5 days. If more than 5 days have passed, skip it and resume your regular schedule.
Wegovy injection (semaglutide) If your next scheduled dose is more than 48 hours away, take the missed injection as soon as possible. If it's less than 48 hours away, skip the missed dose. After two or more consecutive missed doses, contact your provider about restarting.
Mounjaro (tirzepatide) Take the missed injection within 4 days (96 hours). If more than 4 days have passed, skip it and resume your regular schedule.
Zepbound (tirzepatide) Take the missed injection within 4 days (96 hours). If more than 4 days have passed, skip it and resume your regular schedule.

These instructions apply to the specific FDA-approved injectable products listed above. Oral GLP-1 products and compounded medications require separate consideration.

If you're unsure which medication or formulation you're using, contact your prescribing provider or pharmacist before taking another dose.

What Happens If You Miss a Semaglutide Injection?

Semaglutide is the active ingredient in medications including Ozempic and Wegovy.

But even though those products contain the same active ingredient, their prescribing instructions aren't identical.

If You Miss an Ozempic Dose

For once-weekly Ozempic injections, the FDA-approved instructions say you can take your missed dose within five days of your scheduled injection.

If more than five days have passed, skip the missed dose and take your next injection on your regularly scheduled day.

Don't take an extra dose to compensate.

If You Miss a Wegovy Injection

Wegovy injection has a different rule.

If your next scheduled injection is more than two days away, you can take the missed dose as soon as possible.

If your next scheduled dose is less than two days away, skip the missed injection and return to your regular schedule.

If you've missed two or more consecutive weekly doses, contact your provider before restarting. Depending on the interruption and your circumstances, restarting at a lower dose may be necessary to reduce gastrointestinal side effects.

The takeaway? Even when medications share the same active ingredient, you shouldn't assume their instructions are interchangeable.

What Happens If You Miss a Tirzepatide Injection?

Tirzepatide is the active ingredient in Mounjaro and Zepbound.

For these FDA-approved weekly injections, the missed-dose instructions are the same.

If it's been four days (96 hours) or less since your scheduled injection, take the missed dose as soon as possible.

If more than four days have passed, skip that injection and take your next dose on your regularly scheduled day.

The prescribing information also allows patients to change their weekly injection day, provided there are at least 72 hours between doses.

But if you've missed several injections or have been off treatment for weeks, don't assume that taking your previous dose is automatically appropriate.

That's a conversation to have with your provider.

What If You're Taking Compounded Semaglutide or Tirzepatide?

This is especially important for patients receiving medication through a compounding pharmacy.

Don't automatically apply brand-name missed-dose instructions to a compounded GLP-1 prescription without checking with your medical team.

Compounded medications are not FDA-approved products, and their formulations, concentrations, labeling, and dosing instructions can differ.

Some are supplied in multi-dose vials rather than prefilled injection pens.

That creates another important consideration: the amount of medication in milligrams is not the same thing as the number of units drawn into a syringe.

Your provider and dispensing pharmacy should be able to confirm:

  • The medication and concentration you're using
  • Your prescribed dose
  • How that dose is measured
  • Your injection schedule
  • What to do if you miss an injection
  • Whether your dose needs to change after a treatment interruption

Never guess at a dose or try to calculate a replacement dose on your own.

If you aren't sure what to do, ask before injecting.

Will Missing One GLP-1 Dose Affect Your Weight Loss?

One missed GLP-1 injection doesn't necessarily erase your progress or mean your treatment has stopped working.

These medications remain in the body for some time after an injection. Semaglutide has an approximate half-life of one week, while tirzepatide has an approximate half-life of five days.

That means the medication doesn't simply disappear the moment you miss your scheduled injection.

However, drug levels decline over time, and your experience may change.

Some patients notice more hunger.

Some notice food noise returning.

Others may not feel much different after one missed dose.

The response varies.

What I don't want patients doing is trying to compensate for a missed injection by taking more medication than prescribed.

One interruption isn't a reason to rush your next dose or increase it.

Can Your Appetite Come Back After Missing a GLP-1 Dose?

Yes. Some patients notice an increase in hunger or cravings after missing injections, particularly when treatment has been interrupted for longer.

You may notice that you're thinking about food more often or that your usual portions no longer feel as satisfying.

That doesn't automatically mean the medication has permanently stopped working.

It may reflect changes in medication exposure during the interruption.

And it doesn't necessarily mean you need a higher dose.

If your appetite has changed after missing treatment, let your provider know.

We want to understand what happened before deciding what should happen next.

What Happens If You Miss Two or More GLP-1 Doses?

Missing multiple consecutive GLP-1 injections deserves more attention than missing one dose.

After a longer interruption, the amount of medication remaining in your body may be substantially lower.

And depending on your medication, previous dose, treatment history, and how long you've been off it, restarting at the same dose may increase the risk of gastrointestinal side effects.

Remember, these medications are generally started at lower doses and gradually increased for a reason.

The gradual increase helps your body tolerate treatment.

If you've been off medication for several weeks, we need to consider whether jumping back to your previous dose makes sense.

Sometimes it doesn't.

That's why I recommend contacting your prescribing provider before restarting after an extended break.

Can You Restart a GLP-1 at the Same Dose After a Break?

Sometimes, but not always.

There isn't one restart rule that applies to every GLP-1 medication or every patient.

Your provider may need to consider:

  • Which medication you're taking
  • How many doses you've missed
  • Your previous dose
  • How long you were taking that dose
  • Whether you previously experienced side effects
  • Why treatment was interrupted
  • Your current health and other medications

Someone who missed one injection after months of stable treatment may need a different plan from someone who hasn't taken their medication in six weeks.

That's why I don't want patients thinking:

I was taking this dose before, so I should be able to pick up exactly where I left off.

Your previous dose isn't automatically your appropriate restarting dose.

What If You Stopped Taking a GLP-1 for a Month or Longer?

If you've been off semaglutide or tirzepatide for a month or longer, contact your healthcare provider before restarting.

At that point, we're no longer talking about simply being a day or two late.

We're talking about reintroducing treatment after a meaningful interruption.

Your provider may need to reassess your medication, dosing schedule, tolerability, current health, and treatment goals.

And if you've regained some weight or noticed your appetite returning, that should be part of the conversation too.

But I want to make one thing clear:

Taking a break doesn't mean you've failed treatment.

Life happens.

People travel. Financial circumstances change. Schedules get busy. Sometimes patients stop because they're having side effects or want to see how they do without medication.

Whatever the reason, I'd much rather you reconnect with your medical team than guess how to restart.

Can You Double Your GLP-1 Dose If You Miss an Injection?

No. Don't double your GLP-1 dose to make up for a missed injection.

Taking extra medication or taking doses too close together can increase the risk of adverse effects.

This is especially concerning with medications that can cause nausea, vomiting, diarrhea, and other gastrointestinal symptoms.

If you've accidentally taken too much medication, contact your healthcare provider or Poison Control promptly for guidance. Seek emergency care for severe symptoms.

A missed injection should be handled according to your medication's prescribing instructions—not by trying to catch up with extra medication.

Can You Change Your GLP-1 Injection Day?

For some weekly GLP-1 medications, yes.

But the rules vary.

For example, FDA-approved tirzepatide products such as Mounjaro and Zepbound allow changes to the weekly injection day as long as at least 72 hours separate doses.

Ozempic's prescribing information permits changing the injection day when at least 48 hours separate doses.

If you're changing your schedule because of travel, work, or another commitment, check your specific medication's instructions and confirm with your prescribing team when needed.

Don't assume every weekly injection follows the same scheduling rules.

What If You Miss Your Daily GLP-1 Pill?

Not every GLP-1 medication is a weekly injection.

Some semaglutide products are taken orally every day.

For FDA-approved daily semaglutide tablets, including Rybelsus and oral Wegovy, the missed-dose instructions generally direct patients to skip the missed tablet and take the next dose the following day.

Don't double up on tablets to compensate.

Because oral and injectable formulations have different instructions, make sure you're following the guidance for the exact product you were prescribed.

When Should You Call Your GLP-1 Provider After Missing a Dose?

I recommend contacting your provider if:

  • You've missed multiple consecutive injections
  • You've been off treatment for several weeks
  • You're unsure whether to skip or take a missed dose
  • You're considering restarting your previous dose after a break
  • You don't know your medication concentration or prescribed dose
  • You've experienced significant side effects
  • Your appetite or weight has changed substantially during the interruption
  • You've accidentally taken too much medication

If you have diabetes, missed treatment can also affect blood glucose management. Follow your diabetes care plan and contact your clinician for guidance about glucose monitoring and other medications.

And if you develop severe or persistent abdominal pain, repeated vomiting, signs of significant dehydration, or other serious symptoms, seek prompt medical evaluation rather than waiting for a routine follow-up.

The goal isn't to make patients afraid of missing a dose. It's to make sure they know what to do when it happens.

How to Avoid Missing Your GLP-1 Injection

A few simple habits can help.

Pick a consistent injection day. Choose a day that realistically works with your schedule.

Set a recurring reminder. A phone reminder can be more reliable than assuming you'll remember.

Plan ahead for travel. Check your medication's storage requirements and make sure you understand how to handle time away from home.

Stay ahead of refills. Don't wait until you're completely out of medication to think about your next prescription.

Keep your follow-up appointments. Regular communication with your provider can help prevent interruptions caused by missed visits, unclear instructions, or unanswered questions.

And if your schedule changes, ask your medical team how to adjust your injection day safely.

Why Having a GLP-1 Provider You Can Actually Reach Matters

This is one of the things I wish patients thought about before choosing a weight-loss program.

It's easy to focus on the medication itself.

What does it cost?

How quickly can I get it?

Can it be shipped to my house?

Those are reasonable questions.

But I would also ask:

What happens when something doesn't go according to plan?

What if you miss an injection?

What if you're having side effects?

What if you take a break and want to restart?

What if your appetite changes?

What if you aren't sure whether your current dose is appropriate?

Those are the situations where access to medical guidance really matters.

For some medications, a missed injection can be taken within a specific window.

For others, you may need to skip it.

And after a longer break, your provider may need to reassess your dose before you restart.

What matters is making the next decision safely.

Your weight-loss progress doesn't have to follow a perfect schedule.

And neither does life.

Just make sure that when treatment gets interrupted, you have a medical team helping you figure out what comes next.
